1. PRE-REGISTRATION FORM: Complete and submit the bottom of this form to Ms. Souza if paying with cash or check. No form is required with electronic payment
2. PAYMENT OPTIONS:
○ Cash - submit this form and cash to Ms. Souza
○ Check - (made out to Colfax High School) - submit this form and check to Ms. Souza
○ Electronic payment - Colfax Webstore - Schoolcash Online (no form required)
○ Fee waiver – There is no fee for 11th grade students who are qualified and registered for the 2026/2027 Federal Free/Reduced Lunch Program. (See Ms. Souza for verification)
REFUND POLICY: Colfax High School must order the tests in advance, therefore no refunds will be made after September 24, 2026
